I think you need to take both geophysical and cultural ties into account.
Clearly Australia and New Zealand are so far removed from Europe that it
would be ridiculous to make them part of that continent, but exceptions
could be made in 'border' areas where the vast majority of the population
claim to belong to a particular continent. Examples would be Cyprus, and
the Caucasus and Caribbean regions. In some cases this as already been done
as there's no particular geophysical reason why Iceland should be
considered part of Europe instead of North America.
