A few things to answer both Mike and Jesper -

First of all, the nearest place where I have seen the
Gideon book is a library four blocks away. It's only
because of a cold I haven't been going out more than
necessary and researched the book.
But stuff I already have access to or remember, I will
mention.
Even though Gideon mentions no neutral zone in Ceuta,
one exists. Check out the map at
http://www.ceuta.com/basico/c-plano-ciudad1.html . I
have photocopies of maps of Ceuta and Mellila that
also shows the zones. There is no zones for the other
rocks and islands.
The enclave NW of Fujairah in the UAE map Mike found
on the web IS Madha. However, I checked pretty hard
on the web and in the library, but there is no map of
Nahwa. The circular dot Mike mentioned in the UAE
division map is Manama, a local enclave of Ajman. I
actually have not been able to find any map on the web
that even shows all the local UAE enclaves.
The smallest individual border is hard to figure, but
I would guess it is the Penon de Velez de Gomera and
Morocco border. The smallest total border is either
in the Southern Africa quadripoint area or
Gibraltar-Spain, now that Macau is part of China.
There now seems only four countries with enclaves of
second degree. In Baarle, Madha and the
Bangladesh-India area. The third degree enclave in
Bangladesh remains unique. I was wondering if anybody
is aware of second, third or higher degree enclaves in
historic times. The only other one I found was in
Berlin.
There is a International Boundary Study booklet on
Angola-Zaire border. That also may have more
information on the Cabinda tripoint. I have a map of
the boundary, but unfortunately I didn't bother to
photocopy the text. The series should be in any big
University or City Library, though most likely not all
editions.
