what do you mean by why

likely they are both just flat islands close enough to italy to have
each been named flat island in the italian language of the time in
which they were named
independently & without relation to each other

other flat italian islands go by the name of isola piana
using the more usual modern form of the italian word for flat

20 islands in the usa are also named flat island
11 of them scattered around alaska
etc etc
